Membership Cost
How much does a chiropractic membership cost in Lakewood Ranch?
Adult plans range from $65 to $180 per month and child plans from $30 to $75 per month. The difference between tiers is visit frequency. Pricing is identical at all three offices.
Hornback Wellness Membership plans and monthly pricing
| Plan | Visit cadence | Adult, age 13 and up | Child, ages 0 to 12 | Included at every tier |
|---|---|---|---|---|
| Essential | Monthly adjustments | $65 per month | $30 per month | Annual exam, X-rays when exam findings indicate |
| Active | Bi-weekly adjustments | $115 per month | $50 per month | Annual exam, X-rays when exam findings indicate |
| Optimal | Weekly adjustments | $180 per month | $75 per month | Annual exam, X-rays when exam findings indicate |
Which tier fits is a clinical decision, not a shopping decision. As the practice puts it, “The right cadence is a clinical recommendation from your doctor, not a menu pick.” Your monthly amount, including any family discounts, is discussed at intake before anything begins.
Direct Pay Chiropractic
What is a direct pay chiropractor, and how is it different from using insurance?
Direct pay means you pay the practice a set monthly amount for your care instead of the practice billing an insurance company visit by visit. There is no insurance filing on membership care. As the membership materials put it, membership “is between you and your doctor.”
The two approaches are not mutually exclusive. The practice states that “Existing insurance patients and direct-pay patients welcome.” For care outside the membership, the practice is in network with Aetna, Meritain, Medicare, MultiPlan/PHCS, and the VA Community Care Network.
Direct pay membership compared with insurance based chiropractic care
| Consideration | Hornback Wellness Membership | Insurance based care |
|---|---|---|
| Cost predictability | Flat monthly fee, known in advance | Depends on plan, deductible, and visit |
| Claim filing | None required | Required for each visit |
| Visit cadence | Built into the tier: monthly, bi-weekly, or weekly | Set per visit and per plan |
| Family discounts | 5 to 15 percent for additional adults, 10 to 20 percent for additional children | Not applicable |
| Annual exam | Included at every tier | Verify with your plan |
| X-rays | Included when exam findings call for them | Verify with your plan |
Medicare Coverage
Does Medicare cover chiropractic wellness visits?
Medicare covers chiropractic adjustments that treat an active problem. Medicare does not cover wellness or maintenance visits. That distinction is the single most common question members ask, so here is the precise sequence.
While you are being treated for an active problem, Medicare applies and Hornback Chiropractic & Wellness is in network with Medicare. Once your doctor determines your care has shifted from treating an active problem to maintaining the result, Medicare coverage ends for those visits. The membership exists for exactly that point, so regular care can continue on a predictable monthly amount rather than stopping.
Medicare Advantage plans are administered by other carriers and their terms vary. Call your office and the team will verify your specific plan before your visit.
Family Discounts
Do family discounts apply to a chiropractic membership?
Progressive household discounts
| Household member | Discount |
|---|---|
| Second adult | 5 percent off |
| Third adult | 10 percent off |
| Fourth adult and beyond | 15 percent off |
| Second child, ages 0 to 12 | 10 percent off |
| Third child and beyond | 20 percent off |

Additional Payments
How do I pay for care that the membership does not cover?
Services outside the membership can be billed to insurance where applicable, or paid with HSA or FSA funds, CareCredit, cash, or a major credit card. Payment plans are available for larger courses of care.
On insurance network status, the practice is in network with Aetna, Meritain, Medicare, MultiPlan and PHCS, and the VA Community Care Network. As of August 1, 2026, the practice is out of network with Blue Cross Blue Shield. Cigna and UnitedHealthcare are already out of network. Full details are on the insurance and payment options page.
Membership Fit
Who is the membership a good fit for?
Members tend to be people who already use chiropractic care regularly and want it to be simple. Families keeping a whole household moving. Adults managing long term spinal health. Athletes maintaining function through a season. Patients whose insurance does not cover the kind of ongoing care they actually want, including Medicare patients whose care has moved past treating an active problem.
Whether it fits is decided at intake rather than online. The initial consultation is free, and your doctor recommends the cadence.
